To do a self-analysis of your eligibility for Home Health Care, ask yourself the following questions:

• Have I had any falls in the last year?
• Do I have difficulty managing my respiratory status (wheezing, shortness of breath, lack of adequate oxygen?)
• Do I have difficulty managing my current medication regimen (filling prescriptions, taking as-needed meds appropriately?)
• Do I have any side effects from my chronic illnesses?
• Have I been hospitalized in the last year?
• Have I been to the Emergency Department in the last six months?
• Do I have difficulty with transportation, laundry, cleaning, bathing, cooking, or any other activities of daily living?

If your answer to one or more of these questions is “yes,” then you should at least give Caring Nurses a call for a no-obligation assessment.
